Sounds like wastegate sticking, which occured on my '93 CDE at about the same milage. May be sooted up - in which case a strip and clean (of Turbo side only) needs doing. A full strip down needs a Turbo specialist capable of rebalancing the unit - so go for a PX unit, assuming that you are sure it is the problem.
Try applying a solvent like Plus-gas to disolve deposits from the actuator rod entrance into the wastegate housing first, to clear problem. This is what some Main Dealers do.
Unlikely to be Wastegate actuator, as these are very reliable and are aircraft grade according to the spec I have seen on them.
